Description of problem:
After assigning write permission to an appointment, the last entry cannot be
removed i.e. if, for example assign write permission to user1 and then to user2,
user2 can be removed while user1 cannot (the order of removal is not important).

How reproducible:
Always
Steps to Reproduce:
1. Assign write permission to an appointment to user 1
2. Assign write permission to an appointment to user 2
3. Remove write permission to either user 1 or 2
4. Remove write permission to other user
Actual Results: Write permission to other user cannot be removed
Expected Results: Write permission should be removed
