Patch to fix #789 and #843 (hopefully)

This patch /should/ fix both the wrong qp encoding of headers as well as the
wrongly encoded reply-to and disposition-notification-to header fields by
strictly following RFC 2045, RFC 2047 and RFC 2822.  Some issues remain, in
particular the limitation to western european characters.
